1. Download Donor KYC Form.
2. Fill the form completely.

(a) If the Cheque Send by courier then send it along with KYC
Form to “BH-237, Bhao Rao Devras Colony, Sector-12, Pratap
Vihar Ghaziabad, Uttar Pradesh-201009”.( Kindly mentioned
the A/C No: 41145477110 and sign at the back side of cheque

(b) If the Cheque deposited in the bank directly then mail scan
copy of Cheque and KYC form to
“donations@vivaanfoundation.org”.

1. Download Donor KYC Form.
2. Fill the form completely.
3. Attach Scan/Downloaded Copy of Bank Receipt.
